## Fixed

- **`install-hook --hook=pre-push`**: when pushing a new branch or tag, the
hook compared the working tree against the pushed commit instead of
reviewing the commit itself. It now reviews the tip commit only (against
the empty tree for a root commit). Annotated tags are reviewed as their
commit; a pushed ref that does not resolve to a commit, or whose parent is
missing in a shallow clone, is skipped with a warning. An installed hook
keeps its old script until you regenerate it with
  `commitbrief install-hook --hook=pre-push --yes`.
- **MCP `review` tool**: `min_severity` had no effect. It now trims
`findings[]` in the returned document; the `fail_on` gate still evaluates
the full set, and the text summary notes that lower-severity findings are
excluded. The CLI `--json` output is unchanged and stays unfiltered.
- `github.com/spf13/pflag` and `github.com/muesli/termenv` are now listed as
  direct dependencies in `go.mod`.
